Legionnaires Disease With Focal Neurologic Deficits and a Reversible Lesion in the Splenium of the Corpus Callosum A Case Report and Review of the Literature

چکیده
Patients with Legionnaires pneumonia frequently present with neurologic abnormalities but normal neuroimaging findings. We present a case of a healthy 52-year-old man presenting with severe Legionella pneumonia and a unique constellation of focal neurologic deficits including bowel and bladder incontinence, hemiparesis, and ataxia with progression to somnolence. A transient lesion was identified at the splenium of the corpus callosum onmagnetic resonance imaging. His recovery wasmarked by prolonged limb and oral apraxia, but the patient improved with an early combination antibiotic treatment and the late use of corticosteroids. To our knowledge, this is the ninth case reported in the English literature of a patient presenting with Legionnaires disease with neurologic deficits and a reversible lesion at the splenium of the corpus callosum. We present a review of the literature including previous cases, with a focus on neurologic examination, neuroimaging findings, combination antibiotic therapy, and the use of corticosteroids.

Reversible Focal Splenial Lesion on Diffusion–Weighted MRI in Sulfonylurea Intoxication
Neurological findings of hypoglycemia vary from reversible focal deficits to permanent dysfunction or death [1–3]. According to MRI findings, the cerebral cortex, basal ganglion, hippocampus, internal capsule, splenium of corpus callosum (SCC), and cerebral white matter are the most commonly affected sites in hypoglycemic brain injury [2–4]. Hypoglycemic brain injury is usually reversible, and ...
متن کاملA reversible lesion of the corpus callosum splenium with adult influenza-associated encephalitis/encephalopathy: a case report
INTRODUCTION Influenza virus-associated encephalitis/encephalopathy is a severe childhood illness with a poor prognosis. Adult case reports are rare and, to date, there have been no reports of adults with a mild subcortical encephalopathy with reversible lesions of the corpus callosum splenium. CASE PRESENTATION A previously healthy 35-year-old man presented with acute progressive tetraplegia...
متن کاملFocal lesion in the splenium of the corpus callosum.
BACKGROUND AND PURPOSE Discrete focal lesions in the splenium of the corpus callosum on MR images in epileptic patients have received little attention in the literature. Our purpose was to describe these lesions, which may be related to the toxicity of antiepileptic drugs (AEDs), and to discuss the possible mechanisms of their development. METHODS We examined six patients with epilepsy whose ...
متن کاملSHORT REPORT Reversible corpus callosum lesion in legionnaires’ disease
Legionnaires’ disease is often associated with neurological findings. Despite such findings, computed tomography and neuropathological investigations are typically normal. This report describes a reversible lesion of the corpus callosum identified on magnetic resonance imaging (MRI) in a patient with legionnaires’ disease. MRI may show previously undocumented neuropathology in acute legionnaire...
متن کاملA Reversible Lesion of Corpus Callosum Splenium with Adult Influenza Associated Encephalopathy: a case report
An influenza virus associated encephalopathy is severe and poor prognoses in childhood. Adult case reports were still quite rare. Especially, none of them was presenting quite mild subcortical encephalopathy with reversible lesions of corpus callosum splenium. A previously healthy 35-year-old man was affected by acute progressive tetraplegia, transcortical motor aphasia, and mild dullness of hi...
